✅ Section-by-Section Guide

If you’re renewing a Canadian passport in 2026, the first question most people ask is: “Which form do I actually need?” It’s not as simple as it sounds — there are different forms for different situations, and using the wrong one means your entire application gets returned to you.

This guide gives you the straight answer. The main Passport Renewal Canada Form for most adults is called PPTC 054 — the Adult Simplified Renewal Passport Application. But if you live outside Canada, you’ll need a different form. And if this is your first passport, a completely different one applies.

We’ve based this guide directly on the actual official forms — so every section name, every rule, and every warning you read here is accurate and up to date for 2026.

Bottom LineIf you’re an adult renewing inside Canada and your details haven’t changed, you almost certainly need PPTC 054. Keep reading to confirm your eligibility.
Section 1

What Is the PPTC 054 Form?

The PPTC 054 — officially called the Adult Simplified Renewal Passport Application — is the primary Passport Renewal Canada Form for eligible adults applying inside Canada or the United States.

The word “Simplified” is important. This form is much easier than a first-time application because you don’t need a guarantor, a citizenship certificate, or references. You’re essentially telling the government: “My details haven’t changed — please issue me a new passport.”

💡
One Important Note on ReferencesEven the simplified PPTC 054 does require two references (Section 6) — two people who are not your relatives, are 18+, and have known you for at least 2 years. They may be contacted to confirm your identity but do NOT need to sign anything.
Section 2

Can You Use the PPTC 054 Passport Renewal Canada Form?

Before downloading anything, check this list carefully. The form instructions are very clear: if you don’t meet all the requirements, your application will be rejected and returned to you.

✅ You CAN Use PPTC 054 If Your Most Recent Passport:

  • Was issued in the last 15 years
  • Was valid for 5 or 10 years (not a temporary/emergency passport)
  • Was issued when you were 16 years of age or older
  • Has the same name, date of birth, and place of birth you want on your new passport
  • Has the same gender identifier you want (or has an observation sticker with the updated gender)
  • You are applying inside Canada or the USA (includes Bermuda, American Samoa, Puerto Rico, US Virgin Islands)

❌ You CANNOT Use PPTC 054 If:

  • This is your first Canadian passport — use PPTC 153 instead
  • Your name has changed (marriage, legal change, etc.) — use PPTC 153
  • Your previous passport was issued more than 15 years ago
  • You are applying from outside Canada and the USA — use PPTC 482 or PPTC 040
  • Your passport was a temporary or limited-validity document
  • You want to change your gender identifier and it doesn’t already appear via observation sticker — use PPTC 643
⚠️
Lost or Damaged Passport? You can still use PPTC 054 if your passport is lost, stolen, inaccessible, or damaged — but you MUST also complete and submit PPTC 203 (Declaration concerning a lost/stolen/damaged travel document) alongside it.
Section 3

How to Download the Passport Renewal Canada Form (PPTC 054)

There is only one safe place to get the correct, up-to-date form. Always download fresh — never use a saved copy from months ago.

1
Go to the official Government of Canada website

Visit canada.ca/passport-forms — this is the only official source. Never use third-party websites.

2
Find “PPTC 054 (10-2025)”

Look for the October 2025 version — this is the current valid form as of 2026. The version date appears in the bottom-left corner of the form.

3
Open with Adobe Acrobat Reader

Do NOT open in your browser’s built-in PDF viewer (Chrome, Edge, Safari). Download the file and open it in Adobe Acrobat Reader 9 or newer. Only Adobe allows you to properly fill, save, and print the form fields.

4
Fill digitally, then print

Type all information into the PDF fields on your computer. Once complete, print the form on plain white paper. Do not use coloured or recycled paper.

5
Sign by hand in black ink

After printing, sign both pages with your usual handwritten signature in black or dark blue ink. Your signature must stay within the border box shown on the form. Digital signatures are NOT accepted.

🚫
Never Download From Unofficial SitesMany websites offer “passport form downloads” — these may be outdated versions that will get your application rejected. The form version is shown as PPTC 054 (10-2025) in the bottom corner. Always verify this before submitting.

Where Else Can You Pick Up the Physical Form?

If you prefer a printed copy, you can pick one up for free at any of these locations:

  • Any Service Canada Centre (participating locations)
  • Any Passport Program regional office
  • Some Canada Post locations
Section 4

How to Fill Out the PPTC 054 — Every Section Explained

This is the most important part of the guide. The Passport Renewal Canada Form PPTC 054 has 7 numbered sections plus a declaration. Here’s exactly what to do in each one — based on the actual form.

1

Personal Information

Surname, given names, DOB, address, declaration & signature
⚠️ Most Rejections Happen Here

This is the most critical section. The form itself warns you: “The information you provide in section one must be written exactly as it appears in your previous passport. Otherwise, your application will be rejected and returned.”

Write your Surname exactly as it appears on page 2 of your current passport — same spelling, same order
Enter your Date of Birth in YYYY-MM-DD format (e.g., 1985-03-15)
Select Sex exactly as shown on your most recent passport — F, M, or X
Enter your natural eye colour and height in cm or inches
Write your Current Home Address — your passport will be delivered here if no separate mailing address is given
If you have a different mailing address, fill the second address line — delivery goes to mailing address
Include your email address so IRCC can contact you and so you can check application status online
The Anticipated Date of Travel is optional but helpful — tick “Unknown” if unsure
Sign within the border box — a signature that goes outside the box will be rejected. Sign on BOTH pages of the form.
Write the date and city/province where you signed in the Declaration section
2

Most Recent Canadian Passport

Passport number, issue date, expiry date
Required

You need three pieces of information from your current passport: the passport number, the date it was issued, and the date it expires. If you cannot provide all three, you cannot use PPTC 054.

Enclose your current valid passport with your application — it must be the original, not a copy
Check the box if you want your old passport cancelled and returned to you. If you leave it unchecked, it will be securely destroyed and you won’t get it back
If your passport was lost, damaged, or reported stolen, also complete form PPTC 203 and include it
If your passport is still valid for more than 12 months from today, provide a written explanation of why you’re applying early — otherwise your application may be refused
3

Canadian Citizenship

Only required for specific birth date range
Conditional

This section only applies if you were born outside of Canada between February 15, 1977 and April 16, 1981 inclusive. If you were born in Canada, or born before or after this date range, skip this section entirely.

If born in that range: answer question (a) — were you naturalized? If yes, go to section 4
If not naturalized: answer question (b) — was one of your parents born in Canada? If yes, go to section 4
If neither: you’ll need to provide your citizenship certificate (original if before Feb 1, 2012; original or copy if after)
4

Period of Validity

Choose 5-year or 10-year validity
Required

Simply tick one box — 5-year or 10-year validity. This affects your fee and how long your new passport will be valid.

Adults 16 and over can choose either option — 10-year is better value for most people
Fee: 5-year = $122.50 | 10-year = $163.50 (as of March 31, 2026)
If applying from the USA: 5-year = CAN$190 | 10-year = CAN$260
5

Additional Personal Information

Addresses and occupation in the last 2 years
Required

You need to cover your complete address and occupation history for the past 2 years. This is a security and identity verification requirement.

Section 5A — Addresses: If your current address is the same as 2 years ago, tick “Same as current home address.” If you’ve moved, fill in up to 2 previous addresses with From/To dates in YYYY-MM format
Section 5B — Occupation: Tick all that apply: employed, in school, or other (homemaker, retired, unemployed). Fill in the employer/school name, address, phone number, field, and dates
If space runs out, use additional form PPTC 056
There must be no gaps — your 2-year history must be complete
6

References

Two people who know you — not relatives, 18+, known you 2+ years
Required

Provide two references. Unlike a first-time application, your references do NOT need to sign anything or certify photos. They just need to agree that IRCC can contact them to confirm your identity if needed.

Must NOT be your relatives (spouse, children, parents, siblings, grandparents, in-laws — see canada.ca for full definition)
Must be 18 years or older
Must have known you personally for at least 2 years
Fill in their full name, address, phone, email, relationship to you, and how many years they’ve known you
Inform your references — they may be called by the Passport Program
7

Emergency Contact Information

Mandatory — name and contact of someone to reach in emergencies
Mandatory

This section is mandatory. It’s not used for processing — it’s used if you need emergency assistance while outside Canada. You must sign and date this section too.

Fill in surname, given name, relationship to you, daytime phone, alternate phone, and email
Add their current home address in full
Sign and date this section — your signature is required on both pages of the form
D

Credit Card Information (Section D — Mail Applications Only)

Only fill if applying by mail AND paying by credit card
Optional

This section is found on page 3 of the instructions. If you’re mailing your application and want to pay by credit card instead of money order/certified cheque, fill in your card details here.

Accepted cards: Visa, MasterCard, American Express (including prepaid embossed cards and Mastercard Debit)
Fill in: card type, name as on card, card number, expiry month/year, your name as applicant
Sign the authorization and write today’s date
If applying in person, do NOT fill this section — pay at the counter directly
🚫
Critical Warning Directly from the FormThe PPTC 054 form states clearly: “Type or print in CAPITAL LETTERS using black or dark blue ink.” And: “The information you provide in section 1 must be written exactly as it appears in your previous passport. Otherwise, your application will be rejected and returned.”
Section 5

The PPTC 040 Form — For Canadians Applying Abroad

If you are outside Canada and the USA, you cannot use the PPTC 054. Instead, you will need either the PPTC 040 (Adult Abroad General Passport Application) or, if eligible, the PPTC 482 (Adult Abroad Simplified Renewal).

The PPTC 040 is the more complex of the two. It’s used when you don’t qualify for simplified renewal abroad — for example, if your details have changed, this is your first passport, or your previous passport doesn’t meet renewal criteria.

📌
Check Eligibility for PPTC 482 FirstIf you’re abroad and renewing, check if you qualify for PPTC 482 (the simplified version) before using PPTC 040. PPTC 482 doesn’t require a guarantor and is much simpler. Visit canada.ca/passport to check eligibility.
Section 6

How to Fill Out the PPTC 040 — Key Sections

The PPTC 040 has more sections than PPTC 054 because it requires more identity verification. Here are the key differences you need to know.

1

Personal Information

Name, DOB, sex, address, email, signature
All Pages Must Be Signed

Similar to PPTC 054, but note: you must sign your usual signature on all three (3) pages of PPTC 040 (vs. both pages for PPTC 054).

Write name as you want it to appear in the new passport — this can differ from previous passport if you submit a name change document
Provide your current home address AND mailing address — passport is delivered to the mailing address
If address changes within 30 days of submitting, notify the Government of Canada office where you applied
Sex/gender: if different from previous passport, you need PPTC 643
2

Declaration of Guarantor

Required — must be someone who has known you 2+ years
PPTC 040 Only

This section does NOT exist in PPTC 054. For PPTC 040, you need a guarantor who has known you personally for at least 2 years and meets one of two criteria:

Option A (Passport Holder): A Canadian citizen, 18+, with a valid (or expired within 1 year) 5 or 10-year Canadian passport — they can be anywhere in the world, not necessarily the same country
Option B (Eligible Profession): A dentist, medical doctor, lawyer, judge, police officer, pharmacist, pharmacist, bank signing officer, etc. who resides/works in the area served by the Canadian office where you apply
The guarantor must sign the form, write how long they’ve known you, sign the back of one photo, and sign copies of your ID documents
The four bold fields (years known, signature, date, signed at) must be completed by the guarantor themselves — you cannot fill these
No eligible guarantor? Use PPTC 132 (Statutory Declaration in Lieu of Guarantor)
3

Previous Canadian Travel Document

Passport number, issue date — and return preference
Required
Enclose your most recent Canadian passport — valid or expired
Any damaged or previously reported lost/stolen passport will not be returned — it will be securely destroyed
Tick “Yes, please return it to me” if you want your old cancelled passport back
4

Proof of Canadian Citizenship

Birth certificate or citizenship certificate
PPTC 040 Only

This section is unique to PPTC 040 and does not appear in the simplified PPTC 054.

Born in Canada: Submit original Canadian provincial/territorial birth certificate OR citizenship certificate
Born outside Canada: Submit citizenship certificate (original if before Feb 1, 2012; original or copy if after). Other options: certificate of naturalization, registration of birth abroad
Copies of citizenship certificates are accepted if the certificate number starts with K or X
Your proof of citizenship document will be returned to you
8

References

Two references — not relatives, 18+, known you 2+ years
Required

Same reference requirements as PPTC 054 — two people, not relatives, 18 or over, who have known you at least 2 years.

Fill in full name, address, phone, email, relationship, and number of years known
A guarantor and reference can be the same person (they can serve both roles)
9

Emergency Contact Information

Mandatory in both forms
Mandatory
Provide name, relationship, two phone numbers, email, and full address of your emergency contact
Sign and date this section — required on all three pages
Section 7

Passport Photo Requirements (2026)

Photo rejection is the number one reason passport applications get delayed. Both PPTC 054 and PPTC 040 have detailed photo instructions — and there are key differences between them.

📸
Key Difference: PPTC 054 vs PPTC 040 PhotosFor PPTC 054 (simplified renewal inside Canada): No signature is required on the photos. For PPTC 040 (abroad general application): the guarantor must sign the back of ONE photo and write “I certify this to be a true likeness of [your name].”

✅ Photo Requirements (Both Forms)

  • Taken by a commercial photographer in person
  • Professionally printed on photographic paper
  • Frame: 70mm tall × 50mm wide
  • Face height: 31mm to 36mm
  • Clear, sharp, in focus (colour or B&W)
  • Neutral expression, mouth closed
  • Eyes open, looking straight at camera
  • Uniform lighting — no shadows or flash glare
  • White or light-coloured background
  • Taken within the last 6 months
  • Natural skin tone, no alterations
  • Full front view, head not tilted

❌ What Makes Photos Unacceptable

  • Smiling or open mouth
  • Tinted glasses or sunglasses
  • Red-eye effect
  • Prescription glasses with glare (eyes not visible)
  • Hats or head coverings (except religious/medical)
  • Shadow on face or background
  • Photo taken from an existing photo
  • Photos printed at home
  • Stick-on labels on the back
  • Photos older than 6 months
  • Third-party editing software used
  • Wrong dimensions
⚠️
What Must Appear on the Back of One Photo (PPTC 040 Only)The photo studio name and complete address (not a P.O. Box), the date the photo was taken, and the guarantor’s written statement “I certify this to be a true likeness of [applicant’s name]” plus the guarantor’s signature. Stick-on labels are not acceptable — information must be stamped or handwritten.

🏢 Applying In Person (PPTC 054)

Section 10

9 Common Mistakes That Get the Passport Renewal Canada Form Rejected

These are the real mistakes people make — sourced directly from the actual form warnings and common rejection reasons.

1
Name in Section 1 Doesn’t Match Old Passport

The form warns this directly: your name must be written EXACTLY as it appears in your previous passport — same spelling, same order. Even one letter different = rejection and return.

2
Signing Outside the Border Box

Your signature must stay within the printed border box on the form. Signatures that cross the border lines are a common cause of rejection. The form shows a “Correct” vs “Incorrect” example.

3
Not Signing Both Pages

PPTC 054 requires your signature on BOTH pages. PPTC 040 requires signature on all THREE pages. Many people sign only the first page and miss the signature at the bottom of page 2 (emergency contact section).

4
Using an Outdated Form Version

The current version is PPTC 054 (10-2025). If your downloaded form shows an older date in the bottom corner, it may no longer be accepted. Always download fresh from canada.ca before applying.

5
Wrong Date Format

All dates must be in YYYY-MM-DD format. Writing “March 15, 1985” or “15/03/1985” is wrong. It must be “1985-03-15”. This applies everywhere dates appear on the form.

6
Not Ticking the “Return Passport” Box

In Section 2, if you want your old cancelled passport returned to you, you MUST tick the checkbox. If you leave it blank, your passport will be destroyed and not returned — even if it has valid visas inside.

7
Sending Photocopies Instead of Originals

Your current passport must be the original — not a photocopy, not a scan. IRCC will cancel it and either return or destroy it. Copies are never accepted as a substitute.

8
Gaps in the 2-Year Address/Occupation History

Section 5 must cover your complete last 2 years — no gaps. If you moved three times, list all addresses. If space runs out, attach PPTC 056. IRCC may follow up if there are unexplained gaps.

9
Applying More Than 12 Months Before Expiry Without Explanation

If your current passport still has more than 12 months of validity remaining, the form requires a written explanation of why you’re applying early. Without it, IRCC may refuse the application.

Section 11

Frequently Asked Questions About the Passport Renewal Canada Form

What is the form number for the Passport Renewal Canada Form?
The main Passport Renewal Canada Form for adults renewing inside Canada or the USA is PPTC 054 — officially called the Adult Simplified Renewal Passport Application. The current version as of 2026 is PPTC 054 (10-2025). If you’re abroad, you’ll need PPTC 482 (simplified) or PPTC 040 (general application).
Where do I download the PPTC 054 form?
Download it directly from canada.ca/passport-forms — the only official source. Open the PDF in Adobe Acrobat Reader (not your browser’s viewer). Fill it digitally, then print and sign in black ink. Always re-download a fresh copy — do not use forms saved from months ago.
Do I need a guarantor for the PPTC 054 renewal form?
No. The PPTC 054 simplified renewal form does NOT require a guarantor. This is one of the biggest benefits of the simplified renewal process. However, you do still need to provide two references (Section 6) — two non-relatives who are 18 or older and have known you for at least 2 years.
Can I fill out the Passport Renewal Canada Form on my phone or tablet?
Not recommended. The PPTC 054 is a fillable PDF that works best in Adobe Acrobat Reader on a computer. Mobile PDF viewers and browser-based viewers often don’t support the form fields correctly, which can cause your data to be lost when printing. Download Acrobat Reader on a laptop or desktop for best results.
What if I make a mistake on the form — can I use white-out or correction fluid?
No. Do not use white-out, correction fluid, or any covering material on the form. If you make a mistake, print a new clean copy and start over. Because the form can be filled digitally on your computer, it’s easier to just correct it before printing rather than trying to fix a printed copy.
My name changed after marriage — which form do I use?
You cannot use PPTC 054 if your name has changed. You must use PPTC 153 (Adult General Passport Application) instead, and submit a legal name change document such as a marriage certificate. Your new passport will reflect your updated legal name.
Is there an expiry date on a submitted passport application form?
Yes. The form must be completed and signed within the last 12 months before IRCC receives it. If you filled and signed the form more than 12 months ago, you need to start fresh with a new printed form and new signature date.
Can someone else submit the Passport Renewal Canada Form on my behalf?
Yes — an acceptable third party can submit the application for you at a Service Canada Centre or passport office. However, for urgent or express service, you must apply in person yourself. Visit canada.ca/passport-third-party-application for full information on who qualifies as an acceptable third party.
Do I print the form double-sided or single-sided?
Print single-sided. Each page of the form should be on its own separate sheet of paper. Do not print double-sided. Submit all pages together in the correct order with your application package.
