The form warns this directly: your name must be written EXACTLY as it appears in your previous passport — same spelling, same order. Even one letter different = rejection and return.
Your signature must stay within the printed border box on the form. Signatures that cross the border lines are a common cause of rejection. The form shows a “Correct” vs “Incorrect” example.
PPTC 054 requires your signature on BOTH pages. PPTC 040 requires signature on all THREE pages. Many people sign only the first page and miss the signature at the bottom of page 2 (emergency contact section).

Your current passport must be the original — not a photocopy, not a scan. IRCC will cancel it and either return or destroy it. Copies are never accepted as a substitute.
Section 5 must cover your complete last 2 years — no gaps. If you moved three times, list all addresses. If space runs out, attach PPTC 056. IRCC may follow up if there are unexplained gaps.
If your current passport still has more than 12 months of validity remaining, the form requires a written explanation of why you’re applying early. Without it, IRCC may refuse the application.
